Forbearance Forms
Incorrect or incomplete forms will delay your request, so be sure to:
- Choose the correct forbearance form associated with your loan program(s) and servicing center.
- Check the correct boxes and provide all necessary information and signatures on your form.
- Return the signed form to your servicing center's correspondence address (the name of your servicing center appears on your bill).
Federal/FFELP (Title IV) Loan Forbearance Forms

AES Department of Defense Loan Repayment Program Forbearance Request
Department of Defense Loan Repayment Program Forbearance This type of forbearance is granted to borrowers who are eligible to have either all or part of their student loans repaid by the Department of Defense (DOD).
AES Hardship Forbearance Request
Economic Hardship Forbearance This type of forbearance is granted to borrowers who have excessive student loan debt burden. For purposes of this forbearance, excessive student loan debt is defined as having Federal student loan payments that are greater than or equal to 20 percent of the borrower's monthly gross income.
Temporary Hardship Forbearance This discretionary forbearance option is granted to borrowers who intend to repay their student loan(s) but show that repaying them at the present time would constitute a hardship, such as financial difficulty.
AES In-school Forbearance Request
In-school Forbearance This discretionary forbearance option is granted to borrowers experiencing financial difficulty while attending school but who do not meet the criteria to qualify for a school deferment.
Dependent Student Temporary Hardship Forbearance This discretionary forbearance option is granted to PLUS loan borrowers experiencing financial difficulty or hardship when the dependent student, for whose education the loan was disbursed, is enrolled at least half-time at an eligible school.
AES Internship/Residency Forbearance Request
Internship/Residency Forbearance This type of forbearance is granted to borrowers who have used their maximum 24 months of Internship/Residency deferment time or are not eligible to receive an Internship/Residency deferment.
Child Care Provider Loan Forgiveness Forbearance Request
Child Care Provider Loan Forgiveness Forbearance This forbearance is granted to borrowers who perform qualifying child care for the federal loan forgiveness program.
Teacher Loan Forgiveness Forbearance Request
Teacher Loan Forgiveness Forbearance This forbearance is granted to borrowers who perform the qualifying teaching service for the federal loan forgiveness program.
